Lifestyle Management Program
Healthy Monroe’s Lifestyle Management program begins with the Health Risk Evaluation, which is completed by members themselves, and a health screening. These tools are used to identify whether members are candidates for lifestyle management coaching provided by a counselor. The Health Risk Evaluation can be easily accessed by following a link from www.Healthy-Monroe.com.
Disease Management Program
Healthy Monroe’s Disease Management program uses pharmaceutical and medical data to identify members that have any of the 20 chronic diseases that Healthy Monroe manages. A Healthy Monroe nurse will then call the member and set goals to help them optimally manage their conditions.
Healthy Monroe's Personal Self-Care Plan Helps Control Rising Healthcare Cost
Participants monitor their own pertinent health data and report to their Healthy Monroe nurse. Each participant is assigned a primary contact to work with them while participating in Healthy Monroe. They begin by developing a personal self-care plan individualized to deficits regarding their condition. One-on-one feedback is provided regarding their personal self-care plan and modifications are made as needed. In this way, the physician's plan of treatment is enhanced by Healthy Monroe. Participants who are involved in their health care and adhere to treatment plans can improve health, delay complications from chronic conditions, and do their part in controlling rising health care costs.
